Our client Cambridgeshire county council is looking for an Adults Social Worker to join their Learning Disabilities team.
Key Responsibilities
Complete Care Act 2014 assessments, including complex and high-cost care packages
Undertake timely reviews following changes in individuals' needs or circumstances
Deliver a minimum of two completed and recorded assessments per week
Carry out face-to-face assessments and reviews
Actively involve advocates, carers, and families
Analyse and evaluate care and support arrangements
Work closely with brokerage and contracts colleagues
Ensure care packages maximise independence and wellbeing
Maintain accurate and defensible records
Essential Requirements
Qualified Social Worker with Social Work England registration
Experience with Learning Disabilities/Autism
Knowledge of Care Act 2014 and Mental Capacity Act
Experience reviewing complex care packages
