Key Responsibilities
Carry out holistic assessments of children and young people with disabilities in their home environments.
Asses, recommend and provide specialist equipment to support daily living and independence i.e. specialist seating, moving and handling, bathing and toileting.
Assess for and contribute to home adaptations, ensuring environments are safe, functional, and fit for purpose.
Work closely with families and carers, providing guidance, reassurance, and practical support.
Collaborate with multi-disciplinary professionals across
Council services
Health (e.g., NHS colleagues)
Education providers
Partner and voluntary agencies
Contribute to care planning, ensuring interventions are child-centred and outcome-focused
