Overview

Responsibilities
Provide holistic social work support to clients experiencing unauthorised challenges, ensuring their safety and well-being.
Conduct thorough assessments to identify client's needs, strengths, and barriers.
Develop and implement personalised care plans in collaboration with clients and multidisciplinary teams.
Advocate for client's rights and access to services, including healthcare, housing, and legal assistance.
Maintain accurate and confidential case records in compliance with organisational and legal standards.
Build and maintain partnerships with community organisations, agencies, and stakeholders.
Deliver culturally sensitive support and interventions that respect the diverse backgrounds of clients.
Participate in ongoing professional development and team meetings to enhance service delivery.
Qualifications
Bachelor's or Master's degree in Social Work or related field.
Valid social work license or eligibility for licensee.
Experience working with individuals facing unauthorised or precarious situations preferred.
Strong understanding of social justice, equity, and inclusion principles.
Excellent communication, interpersonal, and advocacy skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Commitment to confidentiality and ethical social work practices.
Cultural competency and sensitivity to diverse populations.
Day-to-Day
Engage directly with clients to provide support, counselling, and resource referrals.
Collaborate with healthcare providers, legal professionals, and community services to coordinate comprehensive care.
Document client interactions and update case management systems promptly.
Attend team briefings, training sessions, and community outreach events.
Assess and respond to emergency situations with professionalism and empathy.
