Band 7 / Band 8 Clinical Pharmacist
Main Duties and Responsibilities
Provide specialist pharmaceutical advice to medical, nursing, and pharmacy staff on areas including therapeutic drug selection, supportive care, safe prescribing, drug formulations, and methods of administration. Ensure patients receive high-quality pharmaceutical care throughout their inpatient stay and at discharge, promoting the safe, effective, and cost-efficient use of medicines.
Act as a Non-Medical Independent Prescriber (where qualified), working within agreed clinical competencies and scope of practice. Responsibilities may include prescribing and reviewing medicines, supporting medicines optimisation at admission and discharge, initiating and monitoring treatment plans, and producing discharge prescriptions in accordance with Trust policies and professional standards.
Lead and coordinate the day-to-day delivery of clinical pharmacy services within Acute Medicine, while contributing to service development and improvement. Work collaboratively with the Associate Chief Pharmacist – Clinical Services, clinical teams, and senior managers to ensure the provision of safe, clinically effective, and cost-efficient medicines management.
Develop, implement, monitor, and evaluate clinical governance initiatives relating to pharmacy services within Acute Medicine. Support risk reduction, quality improvement, and the safe and appropriate use of medicines to enhance patient outcomes and maintain high standards of care.
Skills & Experience:
GPHC
Previous NHS experience working as a Clinical Pharmacist
IP preferred but not essential
Strong IT skills, including Microsoft Word, Excel, and PowerPoint
Experience using hospital systems
Excellent organisational and communication skills
