Adult Practitioner
Location: Telford & Wrekin
Working Arrangement: Hybrid – Office based on Mondays, with home and community-based working
We are seeking an experienced Adult Practitioner to join a busy and supportive Adult Social Care team in Telford & Wrekin.
This role offers the opportunity to make a meaningful difference by supporting adults to maintain their independence, improve their wellbeing, and access the support they need through strengths-based and person-centred practice.
Key Responsibilities
Complete Care Act assessments, reviews and reassessments
Undertake Carers Assessments and Mental Capacity Assessments
Contribute to Best Interest decision-making processes
Develop outcome-focused support plans that promote independence
Identify and manage risk, supporting positive risk-taking approaches
Support safeguarding activity and contribute to service improvement
Work collaboratively with families, carers, health professionals and partner agencies
Promote access to community resources, assistive technology and personal budgets
Requirements
Experience working within Adult Social Care
Strong assessment, review and care planning skills
Knowledge of the Care Act, Mental Capacity Act and safeguarding procedures
Enhanced DBS with Adults and Children's Barred List
Full UK Driving Licence and access to a vehicle
