Pay Rate: £16.35 per hour PAYE
Hours: 37 hours per week, Monday to Friday, 9:00am – 5:00pm
Contract: Temporary until 31 August 2026

We are currently recruiting for an experienced Adult Practitioner to support a busy Adult Social Care team. This role offers a hybrid working arrangement, with office attendance required on Mondays and a combination of home working and client visits throughout the week.

### Key Responsibilities

Complete Care Act assessments, reviews and reassessments.

Carry out Carers Assessments and Mental Capacity Assessments.

Develop strengths-based, person-centred support plans.

Promote independence, wellbeing and informed choice.

Support access to community resources, assistive technology and digital solutions.

Manage risk and contribute to safeguarding activities.

Maintain accurate case records and documentation.

Work collaboratively with families, carers, partner agencies and multi-disciplinary teams.

Support individuals to access personal budgets and direct payments.
### Requirements

Experience working within Adult Social Care.
Strong knowledge of the Care Act, Mental Capacity Act and safeguarding principles.
Experience completing reviews, reassessments and support planning.
Excellent communication and assessment skills.
Enhanced DBS with Adult and Child Barred List clearance.
Full UK driving licence and access to a vehicle.
