We are currently recruiting a Support and Wellbeing Worker for our client based in Haringey, the post holder will deliver a need led service both in supported Housing schemes and within the community.
Duties & Responsibilities as a Support and Wellbeing Worker:
* Provide person-centred support to vulnerable adults, promoting independence, dignity, and wellbeing.
* Build positive relationships with service users to understand their individual needs, preferences, and goals.
* Develop, review, and maintain support plans and care plans, ensuring they reflect everyone’s changing needs and circumstances.
* Complete annual support plan reviews and contribute to multi-agency meetings where required.
* Monitor and support mental health and emotional wellbeing, recognising early signs of deterioration and responding appropriately.
* Safeguard service users by identifying and reporting any concerns, abuse, or neglect in line with safeguarding policies and procedures.
* Carry out regular health and safety checks within the service environment to ensure a safe and supportive setting.
* Support with daily living activities, including budgeting, medication prompts, personal care, and access to community services.
* Encourage engagement in social, educational, or employment opportunities to enhance independence and inclusion.
* Maintain accurate records and documentation, including daily notes, incident reports, and risk assessments.
* Work collaboratively with families, healthcare professionals, and external agencies to ensure a holistic approach to care and support.
* Adhere to confidentiality, data protection, and professional boundaries always.
Working Hours: 09.00AM- 17.00PM, Monday to Friday
Requirements for Support and Wellbeing Worker:
* Enhanced DBS Check
* Driving Licence
* Previous Support & Well-being experience
* Experience writing care plans
* Experience supporting vulnerable adults, including those with mental health needs, learning disabilities, or complex support requirements.
* Good understanding of safeguarding principles and how to recognise and report concerns appropriately.
* Knowledge of mental health issues, wellbeing strategies, and person-centred approaches
* Strong awareness of health and safety procedures and ability to carry out routine checks and risk assessments.
* Excellent communication and interpersonal skills, with the ability to build trust and rapport with service users
