Occupational Therapist – Recovery Hubs (A Council in Leeds)

Contract: Locum | Full-time

A council in Leeds is seeking an experienced Occupational Therapist to join the Recovery Hubs service, working as part of a multidisciplinary team supporting adults to regain independence and return safely to their own homes.

The Recovery Hubs consist of Occupational Therapists working alongside Physiotherapists, Nurses, and support staff to deliver a rehabilitation-focused, 24-hour therapeutic approach aimed at promoting independence and positive outcomes for service users.

Main Duties:

Complete holistic Occupational Therapy assessments for adults with a range of needs within a reablement/rehabilitation setting
Develop and implement goal-led, strength-based outcome plans to support independence and recovery
Work closely with support workers and wider MDT colleagues to ensure coordinated delivery of interventions and progress reviews
Lead on discharge planning for service users returning to their own homes
Apply moving and handling expertise, including risk assessment and identification of safe, person-centred solutions
Promote single-handed care principles and reduced dependency where appropriate
Contribute to a 24-hour therapeutic, rehabilitation-focused model of care within the Recovery Hubs
Ensure all interventions are delivered in line with council policies, statutory requirements, and professional standards
Requirements:

HCPC Registered Occupational Therapist
Experience within community, reablement, or rehabilitation settings
Strong assessment skills with a strengths-based, person-centred approach
Confident in moving and handling risk assessment and intervention planning
Experience working within multidisciplinary teams
Ability to manage complex caseloads and support timely discharge planning
